Chronic Disease Self Management Program Leader Training

Do you believe that people should be empowered to manage their own chronic health conditions?

Do you have a chronic health condition or care for someone who does?

Would you like to be a part of a team that facilitates workshops to help people learn skills and strategies to better self-manage their health, so as to enhance their quality of life?

If you answered yes, to any of the above, you may be the perfect leader to facilitate group workshops.  Please consider attending our next training.  The training is free and all teaching materials provided at no charge.

 Dates:      September 18th,19th, 25th and 26th

                               (Thursdays and Fridays)

Times:      9:00 AM – 4:00 PM    (refreshments and lunch provided)

Chronic Disease Self Management – Rescue Mission

Do you suffer from a chronic health condition such as high blood pressure, pain, diabetes, arthritis, heart disease or others?  Are you tired of being tired?  Would you like to learn how to lower the stress and lack of energy associated with your condition?  During this FREE, six-week workshop you will learn a variety of self-management skills to help you “put the life back in your life,” so that you can maintain and/or increase the daily activities of your choice.  What you learn, will enhance the treatment plan recommended by your health care provider.

Workshop Topics Include:

  • Goal Setting
  • Problem Solving
  • Managing Fatigue
  • Healthy Eating
  • Low Impact Exercising
  • Communication effectively with family, friends and your health care team.

This Clinic will be held on Thursdays from 1 p.m. – 3:30 p.m.
