What is GUIDE?
GUIDE (Guiding an Improved Dementia Experience) is a Medicare-funded program for diagnosed dementia patients who participate in Medicare Parts A & B health coverage. Medicare launched the GUIDE program in 2024 to support patients with dementia and their unpaid caregivers as they navigate the challenges of living with dementia. Patients keep their current physicians, and the additional GUIDE personnel provide care in between physician visits.

GUIDE goals:
• Improve quality of life for people living with dementia
• Enhance support for their unpaid caregivers
• Enable people with dementia to remain in their homes and communities longer
What GUIDE Includes:
• Care Navigator provides care for patients, assists caregivers & coordinates with care team
• No cost, copay or deductible for patients
• Focused dementia caregiver education, support and $2,560 of annual respite services
• Personalized care plan including social services and medication recommendations

Who is Eligible for GUIDE?
People with diagnosed dementia and Original Medicare (Parts A & B) as their primary insurance.
GUIDE is not available for dementia patients:
• with Medicare Advantage
• residing in a skilled nursing facility
• enrolled in PACE or hospice programs
Services from Local Office on Aging and Embrace 
Local Office on Aging and Embrace have teamed up to provide integrated social and clinical care to families living with dementia. We provide home telehealth care to help older adults reduce their risks of going to the hospital or the nursing home.
Here’s what we provide:
• Clinical Expertise – Local Nurse Practitioners from Embrace are your Care Navigators
• Local Social Care – LOA identifies and coordinates local resources
• Personalization – We focus on what matters most to the patient and caregiver
• Holistic Approach – We focus on total health, not just the dementia illness
• Telehealth care via video or phone for easy and frequent interactions at home
